As per her mother's wishes, Hazel Grace Lancaster 17 years old she reluctantly joins an online support group for cancer patients. To breathe properly she is using an oxygen tank that she can carry around because of her cancer. She has a conversation with a boy during one of her events and discovers that his name is Augustus Waters. He's there to help Isaac, their mutual friend. Isaac suffered from a canceroustumor within the left eye of his, which was surgically removed. Now he must have the second eye removed. Augustus comes to Hazel after the meeting and says that the tumor is in one eye. He invites Hazel to visit his house and take a look at the movie. The two discuss how they've dealt with their experienceswith breast cancer. Hazel discovers that she is suffering from thyroid cancer. It has been able to spread to her lung. Augustus had been diagnosed with osteosarcoma, and is now cancer-free. Augustus and Hazel decide to share their most loved books before taking Hazel home. Augustus presents Hazel the Price of Dawn and Hazel recommends an Imperial Fliction.

Hazel discusses the beauty of An Implacable Affliction. It's a story about Anna the little girl diagnosed with cancer. It's the only book she's ever read that matches her experiences. She explains how the novel abruptly ends at the end of the sentence leaving the reader without closureas the the fates of the characters. She speculates about Peter Van Houten's identity as the mystery writer of the novel, who she believes was a fugitive from Amsterdam soon after the publication. The author hasn't been heard from since.

Augustus magically informs Hazel that he came across the assistant of Van Houten Lidewij and was successfully able to establish an email correspondence through her. Hazel will share Van Houten's letters with Augustus, and they create a list to ask Van Houten. This is in an attempt to clarify the book's confusing ending. Hazel is concerned regarding Anna's destiny. She believes that Anna's mother is going to be okay and that her parents will be fine after Hazel's passing. Van Houten finally says that he's only able answer the questions of Hazel in person. He invites her visit should she ever be in Amsterdam.

Augustus is invited by Hazel for a lunch soon after. He turns out that he has planned a Dutch-themed picnic. He informs him that Van Houten is a charity that offers wishes to children with cancer will grant his wish. He touches her face and she is thrilled. She realizes that she loves him, however, she is sure that she'll be hurt when she dies. She likens herself to a bomb.

Hazel experiences an intense episodeof the fluid accumulation within her lungs. She is taken in the ICU. After a couple of days, Hazel is released and learns that Augustus has never left the waiting room of the hospital. Hazel is sent a mail from the Van Houten. The letter is more intimate and cryptic. Hazel is now more determined to travel to Amsterdam than ever after having read the letter. However, there's a problem: Hazel's parents and team of doctors don't believe she's physically able to travel. The situation seems to be hopeless until Hazel's doctor Dr. Maria, one of her most experienced doctors, convinces Hazel that she needs to travel in order to live her life.

Augustus, Hazel and Hazel's mother plan to visit Amsterdam. However, the moment Hazel and Augustus encounter Van Houten for the first time for the first time, they find that he's not a brilliant genius, but an unfriendly drunk who doesn't answer Hazel's questions. The two leave Van Houten's house in total disappointment, and are accompanied by Lidewij, who's shocked by Van Houten's behavior. Augustus and Hazel are reunited for a passionate kiss at the end of their tour to the cheers of the spectators. They then return to their hotel, where they are reunited to the first time. Augustus confesses to Hazel that he was scanned by a body scanner while inside the ICU. This revealed that his cancer had returned and spread across. Hazel finds out that Augustus is the grenade that is thrown when they return to Indianapolis. His state worsensand he loses his charisma and confidence. While he gets more vulnerable and fearful but he's still an attractive boy to Hazel. Hazel stops calling him Augustus, and instead refers to him as Gus like his parents. Hazel recognizes how much she cherishes him. Augustus's condition deteriorates quickly. Augustus arranges a funeral as well. Isaac and Hazel offer eulogies. Hazel adopts a line of Van Huyten that talks about the bigger and smaller infinity. She informs Augustus that she is incredibly happy for him and that she wouldn't wish to miss their short time together.

Augustus dies eight days later. Hazel is stunned to see Van Houten at her funeral. Van Houten explained how Gus and he maintained correspondence and that Augustus ordered Van Houten to rectify the mistake he made on his journey from Amsterdam. Van Houten further stated his wife Hazel is required to be present at his funeral. Van Houten reveals Anna's fate abstractly, but Hazel doesn't care. Hazel receives a message from Isaac within a couple of days that Augustus had written something for her. As Hazel attempts to find the letters and finds Van Houten, he appears. He is drunk and reveals that Anna was the name of his daughter. When she was eight years old, she succumbed to cancer. The Imperial Abilities was his literary attempt to reconcile with her passing. Van Houten is instructed by Hazel to drink and write another book.

Hazel eventually learns that Augustus had delivered the pages directly to Van Houten, as he had asked Van Houten to write a thoughtful eulogy about Hazel. Lidewij forces Van Houten to read the book, and then sends them back to Hazel. Hazel finishes the novel by reading the words of Augustus. He explained that, while it's natural to be wounded in this world, we also have the option to choose to let others take care of it. And he was very happy with his choice. He hopes that Hazel will enjoy her decision. Hazel says that she likes the ending of the novel.
